Link Choice is a set of highly discounted health services offered by the area healthcare providers to employers and individuals who cannot afford traditional health insurance coverage. It is not an insurance program.

Link Plus provides discounts on out-of-pocket expenses to beneficiaries of traditional health benefit plans. The program can be purchased by employers or individuals, is inexpensive, simple to administer, reduces costs for the beneficiary, and can greatly reduce premiums for the purchaser. The program can be used with fully-insured and self-funded benefit plans.
Employers and individuals must meet program specific eligibility requirements to participate in the program. For a current list of the fully-insured healthcare plans that support the Link Healthcare program and eligibility requirements, please speak to one of our representatives.
How to Obtain Your Benefits
The following procedures should be used when you seek services from a supporting healthcare services provider:
- First, verify your eligibility on-line using the search function provided below. You will find a member identification number on the ID card provided with this program. Input this number when requested to view your benefit profile.
- You should make the provider aware of the discount before the service is provided.
- The healthcare provider will verify that the member is a current subscriber to the program through this web site.
- The appropriate discounts will be applied to the healthcare services provider’s bill at the time of service.
- In order to gain access to the discounts provided under the Link Healthcare programs you must pay your portion of the bill at the time of service or at least promptly. Detailed information on the payment terms are described in the User Guides provided for each product.
How to Verifty Participation
Individuals that are currently using the program will periodically want to review their profile for current status and download support material for their employees by utilizing the search function provided below.
